Welcome to UniCon. Each and every convention attendee must complete an entry form before playing any sessions at the Convention. Where possible, we ask that you submit team entries in the same envelope.

1. Start by filling out the information above - the parts marked with an ^{*} are required before your entry will be accepted. Please print clearly.
2. In the Scheduling part of the entry form, mark each game you want to play with an X in the session you would like to play it.
3. In the Teams part of the entry form, please list the games you want to play and the team name for each session. If you do not have a full team, please note how many players you need next to the appropriate session. If you don't have a team, please write INDIVIDUAL in the space provided instead.
4. Calculate your total donation to the convention and note that down. Cheques and money orders should be made out to Monash University Roleplayers. Then sign the form.
5. Send your form to:
